I am an out-of-network provider for most insurance plans. At your request, I will provide the forms you need to submit to your insurance provider for reimbursement for your acupuncture treatments. All patients are required to understand what their insurance coverage includes.

Insurance coverage varies from one plan to another – even among plans offered by a single insurer. Contact your insurance provider before treatment to clarify your specific plan’s coverage and reimbursement policies.

important questions you may consider asking your insurance provider:

• Does your specific insurance policy cover acupuncture?
• What is the rate or percentage of reimbursement for acupuncture (in-network and out-of-network)?
• Does your deductible apply to acupuncture treatment? If so, have you met your deductible?
• Does your policy cover acupuncture treatment for your illness/complaint? Or is coverage limited to certain diagnoses? If coverage is limited, what diagnoses are covered?
• Is there a limit to the number of treatments reimbursed? If so, how many and how does the insurance agency calculate those treatments (per illness/complaint or per year)?
• Are there any other limitations to the coverage by your insurance plan for acupuncture treatment?
• How much is the co-pay?
